Understand the deductible before adding cover
A super top-up policy can add health cover above a chosen threshold, called a deductible. The useful question is not just how large the extra cover looks. You need to know which medical expenses count towards the threshold, when it resets and how you would meet costs below it.

What an aggregate deductible means
Many super top-up policies use an aggregate deductible: eligible expenses from more than one claim can count towards a defined threshold during the policy year. The policy wording determines which expenses qualify and whether the threshold applies individually or to a shared family arrangement.
A traditional top-up may instead apply its threshold to each claim. Product names alone are not enough to confirm the calculation. Ask the insurer to show how the offered policy would treat both one large admission and several smaller admissions.
Expenses excluded by the policy should not be assumed to count towards the deductible. A total hospital bill and the amount accepted under the policy may differ. This distinction matters when planning how much you might need to pay yourself.
A simple example with two admissions
This example explains arithmetic only. Assume a ₹5 lakh annual aggregate deductible, enough remaining super top-up cover and two claims in the same policy year. Assume all the stated expenses qualify and there are no other limits, co-payments or deductions.
- First claim: ₹3 lakh. It remains below the ₹5 lakh threshold. The super top-up pays nothing in this example, and ₹2 lakh of the deductible remains.
- Second claim: ₹4 lakh. The first ₹2 lakh completes the deductible. The remaining ₹2 lakh is above the threshold and could be payable by the super top-up under the assumptions.
- Combined position: ₹7 lakh. The deductible accounts for ₹5 lakh; the super top-up calculation accounts for ₹2 lakh.
The ₹5 lakh below the threshold would need to be met through an applicable base policy, employer cover or your own funds, depending on the arrangements. A base policy's payment is also subject to its own terms. Real claims may differ from this simplified example.
Match the threshold to the protection you actually have
Check the current cover available to each person, not just the number on a brochure. An employer policy may include different people, limits or conditions from a personal policy. Ask what happens if that employer cover ends or changes.
Compare the start and end dates of both policies. A deductible defined by the super top-up policy year may not line up with another policy's renewal date. Ask how records from multiple claims are used to show that the threshold has been met.
Do not choose a high deductible only because the premium appears attractive. Consider how you would meet eligible costs below it, plus excluded expenses. This is a discussion about the whole arrangement, including money you may need access to during treatment.
Conditions to compare in a quotation
Waiting periods and medical disclosure
A super top-up has its own terms. Existing base cover does not automatically remove the additional policy's waiting periods or medical questions. Disclose the requested history accurately and ask what continuity benefits, if any, apply to the specific proposal.
Read the exclusions and any conditions attached to acceptance. Tell the insurer about the information requested in its proposal rather than assuming an earlier disclosure to another insurer is enough.
Cover limits and shared family cover
Confirm the super top-up sum insured, who is covered and how the deductible is applied. Ask whether one person's expenses can contribute towards a family threshold and how claims reduce the remaining cover. Check room limits, treatment-specific limits and co-payments where applicable.

Cashless treatment and reimbursement
Ask how a hospital claim is handled when two policies are involved. Check current hospital arrangements, the approval process and which records each insurer needs. Do not assume the hospital will automatically coordinate the two covers or that every cost will be cashless.
Keep copies of bills, discharge documents and settlement records. Before submitting originals, ask how records should be shared if another insurer also needs them. Follow the claim instructions for the policies involved.
What to prepare for the discussion
Start with the number and ages of the people to be insured, their city, the current cover amount and the deductible you are considering. Explain whether the base protection is a personal policy or an employer benefit.
Have these records available when the detailed discussion takes place:
- Current policy schedules and renewal dates.
- The people included in each existing policy.
- The proposed super top-up quotation and wording.
- The medical information requested by the insurer.
- Your preferred hospitals and questions about claim coordination.

No. It is a separate policy arrangement with a deductible and its own conditions. Compare both approaches against your needs and the actual quotations rather than assuming the same headline total means the same protection.
Can several claims count towards the threshold?
They can under an aggregate-deductible arrangement, subject to the policy's definition of eligible expenses and the relevant period. Confirm the wording and whether the calculation is individual or shared before buying.
Does crossing the deductible make every later expense payable?
